Is anyone standardizing limb occlusion pressure for post-op ACL within the first 2–4 weeks? We just finished a 12-week randomized pilot (n=42) using 60–80% Doppler-derived LOP with a 30–15-15–15 scheme and saw about 15% greater isokinetic quadriceps torque at week 8 versus SHAM, but I’m keen to hear how you’re screening contraindications and when you taper once athletes clear a 1.5x bodyweight back squat…
We standardize Doppler LOP early: 50–60% in weeks 1–2, up to about 70% by week 3 if no effusion, paresthesia, or BP spikes. Screen with a quick Wells DVT check plus BP/meds (OCPs/clot history/BMI>35 = start 40–50% and cap at 3 sets), and taper BFR like taking off training wheels once quad LSI hits about 70–75% and hop pain is 0 — drop to 1–2 BFR days while adding 70–80% 1RM; curious if your “15%” bump held after athletes “cleared,” we keep 30–15–15–15 but tighten rest to 30–45 s.
